Purpose of Job This position is responsible for the delivery of individualized patient care through the nursing process of assessment, planning, implementation and evaluation in accordance with the Standards of Nursing Care and Practice as defined by Ridgeview Medical Center, the Minnesota Nurse Practice Standards Act and the ANA Standards of Clinical Nursing Practice. Job Functions Conducts admission and recertification as ordered by the physician. Completes supporting documentation in an accurate and timely manner. Coordinates requests/referrals according to patient need and physician order. Performs supervisory visits and evaluates performance through direct or indirect observation of aides providing care. Advises and educates patients, family members and significant others on health maintenance/status, disease prevention and/or end-of-life care. Serves as a patient advocate in seeking appropriate community resources to meet patient need. Coordinates with and updates the Case Manager in a timely manner. Completes SOC and Quarterly audits to meet the regulatory requirements as requested by manager/PCC. Demonstrates competencies in the specialty area of Home Health. Performs case management functions under the guidance of a supervisor or primary care clinician as assigned. RN Hospice visit nurse with cross training to Skilled Home Health. Performs other duties as assigned.
