The RN Case Manager is responsible to plan, organize and direct the Lifespark services while ensuring compliance with regulatory requirements and Lifespark best practices including but not limited to starts of care, care plan development and ongoing support, client satisfaction, and caregiver supervision, engagement and on-boarding. Qualifications: Registered Nurse, BSN preferred Prefer advanced certification as a Care Manager, Holistic Nursing, Geriatric CM, and/or Gerontology 2 years' RN experience in community-based services (assisted living, home care, hospice, public health) Experience working with seniors preferred Experience in customer service/direct member contact field preferred Experience supervising caregivers Contact Lifespark Now!
